This form is for motor carriers who require the BOC-3 filing and are inactive. Complete this form to authorize SPA to submit the BOC-3 filing AND the reinstatement request on your behalf. Once we receive your submission, we will review your record with the FMCSA, notify you of any necessary filings, and will see that your operating authority turns active. Once your insurance and BOC-3 are on file, reinstatement usually only takes 3-4 days after we submit the reinstatement request to the FMCSA.
