REINSTATEMENT OF AUTHORITY FORM

Please complete our questionnaire to submit your request for reinstatement. Once we receive your request, we will review your record with the FMCSA, notify you of any necessary filings, and will see that your operating authority turns active. Once your insurance and BOC-3 are on file, reinstatement usually only takes 3-4 days after we submit the reinstatement request to the FMCSA.
